Do you want to reserve the entity name?
Name reservations are not required, but if you need time to prepare the paperwork to form a corporation or LLC, and you want to “hold” the proposed name so no one else can take it, then you may want to obtain a name reservation. You can check name availability, read the naming requirements, and reserve the name here.
Prepare and submit the right documents:
Our forms are easy to complete and have separate, detailed Instructions. Read the Instructions to the document you are submitting. Usually, “Attachment” forms are used for adding more persons than the main form has space for, but with LLCs, at least one attachment form may be required. Read carefully and submit the appropriate attachment form with your LLC documents. Below is a guide listing the documents that must be submitted to the A.C.C. in order to create, change, or amend an

· Cover Sheet; and
· Articles of Incorporation (for-profit), together with any attachment forms; and
· Statutory Agent Acceptance form; and
· Certificate of Disclosure, together with any attachment forms.
To form an
· Cover Sheet; and
· Articles of Incorporation (nonprofit), together with any attachment forms; and
· Statutory Agent Acceptance form; and
· Certificate of Disclosure, together with any attachment forms.
To form an
· Cover Sheet; and
· Articles of Organization, together with EITHER a Manager Structure Attachment OR a Member Structure Attachment, plus any other attachment forms; and
· Statutory Agent Acceptance form.
To change a statutory agent name or address or a known place of business address, or to appoint a different statutory agent, submit all of the following to the Arizona Corporation Commission:
· Cover Sheet; and
· Statement of Change for your entity type – use corporation forms for corporations and use LLC forms for LLCs; and
· Statutory Agent Acceptance form, if a new or different statutory agent is being appointed.
To amend Articles of Incorporation of an
· Articles of Amendment for your entity type – use corporation forms for corporations and use LLC forms for LLCs; and
· Any attachments; and
· Statutory Agent Acceptance form, if a new or different statutory agent is being appointed.
· Cover Sheet; and
· Articles of Dissolution, together with any attachments.
To terminate a limited liability company, submit all of the following to the Arizona Corporation Commission:
· Articles of Termination.
